Health Navigator

This position is responsible for assisting clients with referrals to appropriate health services programs and resources.

Job Duties

  • Screens and assesses client needs and connects clients to appropriate resources; provides in-home and other on-site visits.
  • Provides support to clients and communities in the development of communication skills and risk reduction behaviors.
  • Communicates with clients and service provider partners to identify issues and provide solutions.
  • Coordinates with other department staff to review cases with supervisor, to manage a sustainable caseload, and discuss complex client needs.
  • Researches and identifies internal and external resources to meet client needs.
  • Provides information to clients regarding program eligibility requirements.
  • Collaborates with medical offices, nurses, and community partners to provide services to clients.
  • Attends community events such as health fairs to provide program information to the general public.
  • Participates in continuing education.
  • Performs related duties.

Qualifications

Education/Experience

  • High School Diploma or equivalent.
  • One to two years relevant experience in Social Work, Community/Public Health or related field.
  • Bilingual experience preferred, but not required.

Knowledge/Skills/Abilities

  • Knowledge of program eligibility requirements.
  • Knowledge of community resources.
  • Knowledge of case plan management principles.
  • Knowledge of computers and job-related software programs.
  • Skill in problem solving.
  • Skill in prioritizing and planning.
  • Skill in interpersonal relations.
  • Skill in oral and written communication.
